Health Savings Account Tax Forms
Employees who had an HSA in the prior tax year will be receiving HSA Statements—a 1099-SA and a 5498-SA from HealthEquity. If you have questions about the HSA tax forms, please call HealthEquity at 1-866-346-5800.
Update from HealthEquity as of Friday, 04/14/2025: On both the HealthEquity mobile application and HealthEquity website, certain data fields on the 1099-SA and 5498-SA forms for tax year 2024 may have failed to display all relevant data for some account holders. This error impacted primarily mobile app users, though some website users were also affected. Most account holders were not impacted by this error.
This issue has been resolved, and tax forms are now displaying as expected. We are reaching out to potentially impacted account holders to recommend that they compare any previously downloaded 2024 tax forms to those currently available.
1099-SA
- HealthEquity provides the 1099-SA tax form if there have been any distributions from an HSA between January 1 – December 31 of the previous year.
- A distribution can be a payment directly to a medical service provider or a reimbursement to you.
- If there were not any distributions last year, a 1099 form will not be provided.
- The IRS website has a sample form as a useful reference.
- A 1099 form can only cover one distribution type. Certain scenarios, such as distributions of excess contributions, will generate a second 1099 form for the account.
HealthEquity sends the 1099-SA in the mail, postmarked no later than January 31. Tax documents can also be viewed online, via the member portal by following these steps:
- In the HealthEquity member portal, select 'Tax Forms' from the 'Docs & Forms' tab.
- Locate the section for Form 1099-SA.
- Select the tax year you wish to view from the dropdown menu.
5498 Form
- HealthEquity provides a 5498 form if contributions were made to an HSA between January 1 – December 31 of the previous year.
- This includes scheduled payroll contributions, one-time contributions, contributions from your employer and contributions from any other party on your behalf.
- The IRS website has a sample form as a useful reference.
- HealthEquity provides this form for you for your records. You do not need to include it with your taxes.
HealthEquity sends the 5498-SA in the mail. Tax documents can also be viewed online, via the member portal by following these steps:
- In the HealthEquity member portal, select 'Tax Forms' from the 'Docs & Forms' tab.
- Locate the section for Form 1099-SA.
- Select the tax year you wish to view from the dropdown menu.
